At Sacred Heart Hospice Care, we are dedicated to providing compassionate care and support to patients starting from our certified home health aides who provide routine personal care, bathinf, dressing, or grooming patients on scheduled times.

Our Medical Social Workers provide counseling and assesses social service needs of the patient and their family. Assist in coping with anxiety, fear, stress, family conflicts, financial needs, depression and concerns about death.

Our Non-denominational chaplin are integral to the care team, our chaplins provide spiritual support to patients and their families in order to find peace, acceptance, and meaning during pivotal time in their lives.

Our Registered and Licensed Vocational Nurses conduct regular visits on scheduled times and evaluates patient's condition, coordinate plan of care and teaches the family about proper care, use of medication and pain management.

At Sacred Heart Hospice Care our Physician works as a part of the hospice team and remains as the patient's primary physician.

The Medical Director in charge leads the hospice team in developing a plan of care for the patient. Certifies prognosis of attending physician and provides consultations to physician.
